Community Medical Services has delivered evidence-based opioid use disorder treatment since 1983. Today it operates more than 80 outpatient treatment clinics across 12 states, serving thousands of patients daily. For years, every clinic ran its own isolated Methasoft instance, each with its own naming conventions, formatting, and data quirks built up over time.
Getting a unified operational picture meant someone manually pulling exports from each database and trying to reconcile them by hand. At 10 clinics, that is tedious. At 80, it is unsustainable. When CMS made the decision to move to Kipu, the strategic direction was clear. The hard question was how to get there without disrupting daily dosing for thousands of patients who depend on it.
Reimagined Health had already been working alongside CMS for several years before the migration began, building a deep understanding of the organization's workflows, its data, and the operational complexity of running opioid treatment programs at scale.
Solution
One methodology, 80 clinics, zero downtime
Normalizing the unnormalizable. Across 80 Methasoft instances, the same data had been entered, labeled, and formatted in dozens of different ways. Before a single record could move to Kipu, all of it had to be brought into a clean, consistent structure, requiring not just technical skill but a working knowledge of how opioid treatment clinics actually operate.
A proven methodology. Reimagined's Extract Engine and phased rollout approach is purpose-built for the Methasoft-to-Kipu pathway. Patient records, clinical documents, and structured reports are migrated clinic by clinic, with each location's operations maintained throughout. No downtime. No gaps in patient care.
Rebuilding the reporting foundation. Alongside the migration, Reimagined built a Databricks data warehouse and Power BI reporting layer on Kipu data, giving CMS the real-time operational visibility it never had across 80 disconnected databases.
Results
Zero gaps in care, one clinic at a time.
CMS is in the early stages of a phased rollout planned for completion by the end of 2026. Pilot clinics shut down Methasoft at the end of a normal Friday and opened Monday morning running on Kipu, with no gaps in care for patients who depend on daily dosing.
Data validation is built directly into the migration tooling, so integrity issues are caught before a clinic goes live. As each cutover completes, the timeline is getting tighter, not longer, and the methodology improves with every clinic.
Just as critical is what happens after go-live. Reimagined has demonstrated the ability to listen to provider feedback in real time and adjust how data is presented, so staff trust what's on their screen from day one.
